The staff at Smile 32 Dentistry will always do our best to help you to maximize your benefits. Our staff is trained to help answer questions you may have relating to how your claim has been filed or regarding any additional information your carrier may request in order to process your claim. Although we file claims and direct bill your insurance on your behalf as a courtesy, your dental insurance policy is a contract between you, your employer and your insurance company. We are not a party to that contract. Coverage issues can only be addressed by your employer or group plan administrator.
Your treatment plan is individually tailored for you based on the diagnosis of your dental health and is not based on dental insurance benefits or coverage. Not all services that have been recommended are eligible for payment from insurance. In some instances, insurance companies may decide there are services they will not cover. It is your responsibility to thoroughly understand the coverage and exceptions to your particular policy.
As a courtesy to all of our insured patients, we will submit your dental insurance claim forms on your behalf and request that insurance pay the clinic directly. You are responsible at the time of treatment for the payment of any co-insurance portions not covered by your insurance plan. Any payments made directly to you by your insurance company for unpaid balances should be forwarded immediately to our office so that your account can be credited accordingly. Your claim will be filed immediately, and benefits are normally paid by your insurance company within 30 days. The filing of an insurance claim does not relieve you of timely payment on your account. Any unpaid balance not covered by insurance will automatically become your responsibility and a statement will be issued to you for the unpaid amount. The clinic will not assume responsibility for charges incurred that may not be covered by insurance.
